It's not really any kind of planned approach. I'm completely self taught with web programming, so I'm sure I'm doing things less than optimally. I started with just static html pages, then learned how to add some javascript in, then added php to pull from databases and generate pages dynamically. This progression just lead me to add the php inside of already working html pages. I only use php as the base page when I'm using the URL as a variable to generate the desired page.
