In Baldwin's case, the armourer should have loaded the weapon, test fired it, reloaded it, and personally handed the gun to Baldwin. That didn't happen. While working on a movie set, it seems actors have become to reliant on others to do their job. It's surprising that a tragedy like that didn't happen sooner.
