How is it determined when a star is attached? More inside...

So, a couple months back Garry Shandling was not included for Captain America 2 because he wasn't listed in the title sequence with the others (just the end credits scroll) and yet I just watched Jack Ryan: Shadow Recruit and despite David Paymer not being listed with the other cast, just the end credits scroll, and even then, towards the end of it (plus it was a bit part, maybe 2-minutes of screentime, just like Shandling), he's listed.

Just curious how it's determined who gets attached and who doesn't...
