just a thought (spoilers)
It is my understanding a "falling" kick is not always needed to come out of the dream state. As seen at the beginning of the movie Gordon Levitt says to Haas "give him the kick (to Leo)" only after Haas tries to slap him awake and that doesnt work. Therefore waking up from the van dream only requires the sedation to wear off (as long as you arent in limbo like Leo and Saito). Though the scence I described is taking place in a dream anyway, so who knows what rules apply.
