To clarify - (SPOILERS)
Here's what is still unclear...
Joseph Gordon Levitt gets the kick going in the elevator with the explosions. Now everyone wakes up from that dream and they are back in the dream with the van falling off the bridge. Since the van is falling and initiating the kick, upon waking up from the elevator dream, why don't they then wake up as soon as they wake up in the van?
We see the shapeshifting guy swim to shore with Cilian Murphy. So they are still in the dream post-van-falling-off-bridge. They never should have been swimming in the first place. they should have woken up before they could even swim in the river.
